Black Oak Clinic LLC is a rural healthcare clinic owned and operated by a nurse practitioner in Broken Bow, Oklahoma. We serve and focus more on adolescent, adult and gerontological health needs. We take appointments and also accept walk-ins. We are also able to provide home visits for vulnerable patients within 15 miles of the clinic subject to availability. We offer telehealth services (videochat visit).

Broken Bow Health and Rehab offers skilled nursing, inpatient rehabilitation, and long-term care services. This 105 bed facility provides residents with inpatient rehabilitation services, including physical therapy (PT), occupational therapy (OT), and speech therapy (ST).
